I got curious about this yesterday. My own insurance company, Country Financial, will cover musical instruments under a homeowners policy for theft, fire, etc. I imagine policies vary, but my own will allow for up to $2500 for musical instruments. 

However, if one plays professionally, additional insurance can be taken out and added to the homeowner policy. The rate they go by is $1.30 premium per $100 worth of gear, and that will not only cover theft, fire, etc, in the home, but also if gear were get damaged, destroyed, or stolen while the gear was out of the house. So if my car got broken into and my amps & harps stolen, they'd be covered. They do require some itemization, and serial numbers where they apply, but I don't see that as much of an issue myself.

The way I've been calculating value on things is going by MSRP. That way, I figure with the ever-rising prices of harps, I'm probably going to be covered. Same thing goes for current production gear like cables, cases, and some mics. Vintage items like mics, amps, and out of production harps are going to be more difficult to calculate, but I'm working on it.

I pay automatically so I'm not sure if it's still on but when I first signed 
up a few years ago they gave me a discount for subscribing to Acoustic 
Guitar magazine.  I applied by phone and the rep told me about it.  You 
didn't even have to actually have a subscription as long as you said you 
intended to get one.
