It happened to me once a few years back.  I must confess I was blowing too hard on an aged harp - not a custom or in very good condition, so I deserved it, but I wrote a poem about it under my pen name of Joe Russell.  It appears below:

I raised this very question several months ago and got run out on a  
rail...........I guess it wasn't just me........  My specific question was  
with standard MB's........then I had stated that having spoken with others  
that had in fact "sucked reeds down their throat" literally.......one of  
the folks I had heard that it had happened to was James  
Cotton.........(second hand info on this) it would appear that he's had  
enough also after a few decades......................

at the very beginning
of a quick tempo
funky, high-energy 
harmonica solo
I drew inside my mouth
a harmonica reed
crafting my musical statement
as my tongue endeavored to save my life
a true performer does not cease
in the midst of artistic signature
but must instead persevere
no matter the consequence
irregardless of the fact
that a one inch long
one sixteenth inch wide
flat piece of brass
threatened my very existence
while I played soulfully
as my tongue worked to secure
this errant brass dagger
yet the band played on
with no knowledge of my plight
my tongue feverously fishing
the reed deviously darting
as I managed to park the reed
at the outside of a stalwart molar
complex victory was mine
I basked in applause
