1. In my version of the Real Book, circa 1975, The first two chords  
are just Eb- and C-7b5. Maybe the notation in your edition suggests  
the bass line walks down from Eb to C.

2. Bar 6 in my book is just an Ab7, I guess the chord in your book  
could be read as an Ab#11.

3. & 4. who knows?
