1	I occasionally play harp thru a delay.
2	Yes, I tap in a delay rate relative to the tempo of each song:  
quarter notes or eighth notes or half notes or triplets, etc...
3	Sometimes by foot, sometimes by hand...  which ever is most  
convenient at the moment.
4	There is a digital delay built into my Peavey Transformer amp.  The  
delay  rate can be set using foot pedal which came with the amp or  
tapping a buttin on the face of the amp.

A triplet echo against a slow blues is especially nice.
