Easy to make one.
Take one PJ055B
http://www.surplussales.com/Connectors/Aud1-4-1.html
and wire two Twin Hi-D Jax to it
http://www.switchcraft.com/products/jack-87.html
with some shielded wire and shrink tubing.
Or you could put them in a small project box
http://www.circuitspecialists.com/level.itml/icOid/5888
and put a dual on one side, and a single on the other and use a guitar cord
to connect to the amp.
I make these kinds of connectors and patch assemblies all the time. Love
the smell of fresh solder in the morning. =8^D
How much would you pay for a professionally made product?
PEACE

(all with 1/4" male plugs) together to form a "Y" cable, so you can have a
mic cable connected to two amplifiers ?

and 2 1/4" female connectors on the other end.
