if you don't know where it's been...and it isn't new? don't play it.
 
clean it first.
 
there have been tons written here about cleaning a harmonica. If it's a  
chromatic, there have been tons of information written on slidemeister as well  
about how to clean a harmonica.  There are probably youtube videos as well  
where one can learn how to follow the proper procedures.
 
Don't play it until it's clean and rust free.
