The best solution I've found is the BOSE PAS concept - At a meeting with an experienced sound tech, we were debating the in ear monitor vs. the natural sound mix. As high tech as the in ear is, it still disassociates the musicians from what is really going on. Five musicians with five different personal mixes in their ears won't be playing to the unified groove, but to five different sounds. Then, it is up to the front of the house sound man to balance the overall mix. Therefore, the resultant sound will have nothing to do with what and how the musicians are playing, but will have everything to do with the philosophy of the man out front, who tends to change a lot from gig to gig.
 
Better is for the musicians to hear exactly what they play and blend together in real time and space, just like they used to do in the "ol' days". Makes for better musical interplay, deeper understanding in a community spirit from the musicians and a much lower overall volume.
