Note my complete lack of any scholarly research:

If you have a song that goes from the 1 chord to the 5 chord, you'd  
probably chord in 1st position for polka music etc. For those tunes  
that go from the 1 chord to the 4 chord , you'd play cross harp (2nd  
position). That would be just by ear, matching the harp chords with  
the guitar chords. Try a little melody over those chords and you would  
soon discover you can hit the African notes the blue notes that aren't  
quite in our music theory. Probably figure out how to avoid that pesky  
major 7th. After that, if you're Howard Levy you'd be rapidly figuring  
out 13th position to play with double augmented 14th chords.
