What you are describing sounds very much like a Hohner Little Lady -
a four-hole miniature harmonica with a one-octave range.
Several manufacturers make harmonicas like this, including Hohner
(Little Lady), Suzuki (Metal Major Mini), Lee Oskar (Oskamonica),
Huang and Hering.
You could use the reedplates from any of these - this is what a
jeweler cannot make - and have the jeweler make the comb - the part
in the middle between the reedplates, and the engraved covers, and
assemble the whole thing.
