The clue here is that when the progression goes to the IV chord
Portnoy drops down to the A *below* the flat seventh and that note is
off the low end of the D harp.  Indeed, one of the best ways to
examine the use of the half-tone 3 hole draw is to listen to any first
position blues.  Other examples: "Lou Ella" (J.B. Lenoir) from "Swear
To Tell The Truth", by Paul Rishell, Little Annie Raines on harp 
(Tone Cool 1148);  "Hard Headed Woman" from "Can't Keep Lovin' You" by
Walter Horton (Blind Pig BP71484).
