I've used both versions of the ER, the customs and the off the shelf 
model.  Both work well.  The non custom version just pushes in.  For my 
ear canal shape my custom moulded plug doesn't really so much screw in 
as twist in.  It's less than a 1/4 turn.  Ear canal shapes vary, I don't 
know that my experience would generalise to anyone else's.  What I like 
about the ER technology is the quality of the attenuation, flat across 
frequencies with no notching at a particular point.  fjm
