If the tonic is on 1D and 4D, you're playing in third position. On a D harp, that's the blues scale in key of E.

In the Exploring 3rd Position book/CD, David Barrett has a page toward the
end with the 1st through 4th hole D blues scale - 1D 2D" 2D 3D'" 3D" 4B 4D
(or D, F, G, Ab, A, C, D) played on a D harp. But isn't that first position
- D blues scale on a D harp? Just confused by that since it's a book on 3rd
position!
