I don't know if this helps but;
the + (plus) sign in this case is (wrongly IMO) to mean SHARP (#) 11(4)
not ADD 4
On a dominant chord (or any chord with a major 3rd) you would not want to use a 4th or 11th- it will clash with the major 3rd and yes, 
"like we're drifting into another key ahead of time."
 
I think Mr Winxa did answer this already, except at one point he referred to it only as an 11, not a sharp 11.
